While I don't think he was on heroin the whole time, it is possible for someone to be a junky and carry on somewhat normally. This depends on:

1) if they always have access to the drug
2) if they always have the money for the drug
3) if they are mostly just using to stop withdrawal
4) if they are shooting or sniffing

It is possible jim sniffed most of his shit and then went big at night. If he was physically dependent, it is possible he did not have many of the effects of the drug except not being in withdrawal. If he had access and never went without, it's very possible he could carry on normally to everyone who didn't interact with him intimately. The band certainly knew though.
