I'd say usually the safe word is more so the person on the receiving end can get what he or she wants out of it, rather than the person giving the punishment, which could be spanking or shocking or trampling or whatever. Doesn't necessarily have to be a rape fantasy tho.

I mean in the context you mentioned "if you are having sex with a girl" then you're already on the rape train, but you aren't thinking beyond "man penetrates female's vagina with his penis" and your sexual exploration ends there.
